SEE BELOW FOR CORONAVIRUS PRECAUTIONS:
ALL PATIENTS WITH APPOINTMENTS AT OUR MAIN OFFICE
In an effort to further ensure the health and safety of our patients, we are taking the following precautions:
Upon arriving at the office, all patients will be screened and have their temperature checked before entering the office. We are offering patients the option to wait in your vehicle until you are called for your appointment. When you arrive at the office, come sign-in at the screening desk, write your phone number on the sign in strip and we will call you when it is time to be seen by your Provider.
Our office has now implemented a mandatory masking policy. We are asking that, when coming in for an appointment at the office, you bring your own mask from home. We are doing our best to provide masks for all our staff and patients but with current conditions being as they are, shipments can be sporadic. Also, we highly discourage bringing anyone with you to your appointment unless they are your caregiver.
If you have been tested for COVID-19 and are coming into the office for an appointment, please bring your test results with you. We need to have the results prior to you being seen by your Provider.
Thank you in advance for your cooperation!
WE DO NOT HAVE TESTING KITS ON-SITE FOR THE CORONAVIRUS. TESTING CAN ONLY BE PERFORMED AT CERTAIN DESIGNATED FACILITIES.
TO ALL EXISTING AND POTENTIAL OFMC PATIENTS REGARDING THE CORONAVIRUS:
