SCHEDULING A TELEHEALTH VISIT
Please send an email to requesting an appointment to be scheduled via Telehealth.
Please include the following information:
Date of Birth
Reason for Appointment (brief description - 10 words or less)
After you receive a reply email to schedule a date and time, a second email containing a link to the ZOOM meeting, along with the Meeting ID and Password with a confirmation of the date, time, and provider you are scheduled with will be sent to you. The second email will also contain the link for your Telehealth visit.
