This page will give you information on how to schedule a Telehealth visit, supported devices and browsers for the ZOOM Telehealth application, how to login and access your visit/meeting, and more.

*You MUST download the ZOOM application on a device that has the capability to record video AND audio. 

 

 The correct "ZOOM" app that you want to download is the one with this icon:

 

SCHEDULING A TELEHEALTH VISIT

Please send an email to telemedicine@ocalafmc.com requesting an appointment to be scheduled via Telehealth.

Please include the following information:

  1. Name

  2. Date of Birth

  3. Preferred Provider

  4. Reason for Appointment (brief description - 10 words or less)

 

After you receive a reply email to schedule a date and time, a second email containing a link to the ZOOM meeting, along with the Meeting ID and Password with a confirmation of the date, time, and provider you are scheduled with will be sent to you. The second email will also contain the link for your Telehealth visit.

We do request that you join the meeting at least 15 minutes prior to the scheduled appointment time. When logged into the Meeting you may be asked to allow video and audio access – YOU MUST ALLOW BOTH FOR THE VISIT TO OCCUR.

 

YOU CAN ALSO SEND AN APPOINTMENT REQUEST THROUGH THE PATIENT PORTAL TO REQUEST A TELEHEALTH APPOINTMENT. MAKE SURE TO SPECIFY TELEHEALTH IN THE MESSAGE.

ACCESSING YOUR VISIT

The following are the steps you will need to take in order to attend your Telehealth visit.

  1. Go to ofmc.zoom.us page and click “JOIN” or go to your ZOOM appointment email and click the
    Join Zoom Meeting” link.

  2. You will then need to enter your Meeting ID that was sent to you via email, then click “JOIN”.

3.  The next screen will prompt you to enter passcode that was also sent to you via email, then click “JOIN”.

4.  You will then have two options, you can download and run ZOOM (click “RUN” on the download bar at the bottom of the screen) or click “Join from your browser”.

5.  You will then be connected to your Telehealth visit.
