ALL PERSONS 60 YEARS OF AGE AND OLDER ARE ELIGIBLE TO RECEIVE THE VACCINE IN FLORIDA

Florida has many sites providing COVID-19 vaccine. Use the vaccine finder to find a site near you. Supplies are limited and appointments may be required.

 

  • Persons 60 years of age and older

  • Persons determined to be extremely vulnerable by a physician (form required)

            YOUR OFMC PCP CAN COMPLETE THIS FORM FOR YOU.

  • Health care workers with direct patient contact

  • K-12 school employees 50 years of age and older

  • Sworn law enforcement officers 50 years of age and older

  • Firefighters 50 years of age and older

Advent Health COVID-19 Vaccine Information for General Public - January 4th, 2021

Currently, COVID-19 vaccines are not available to the general public at any AdventHealth facility. We encourage the general public to visit www.CoronavirusVaccineFacts.com to sign up for email updates on when vaccines will be available from AdventHealth.
